Please fill out the form below to apply for a grant for COVID-19 revenue loss and expenses. The first part will determine whether or not you are eligible to apply for a grant. Once you determine whether or not you are eligible you will be asked to supply information about your business, how COVID-19 has affected your business, and how we can help.
Please note, you will be required to upload supporting documents—please have them ready before starting the application. Also note, you should consider applying to the Energize Colorado gap fund for their $15,000 grant (reopening shortly for a second round) at the same time. Both applications ask similar questions.
Required documents may include Bank statements, Financial ledgers, Point of sale revenue reports, Tax filings, and Other financial records. If you request money for expenses already incurred due to COVID-19, you will be asked to submit relevant documentation of increased operational costs to control the Covid-19 public health emergency since March 1, 2020 to date of application, including Expense receipts or Proof of payment for expenses. If you have already applied, you can append it with additional months losses and expenses to increase the amount of your grant up to the limit here.
